This position is for a Home Health Occupational Therapist responsible for providing occupational therapy services to patients in West Portland, Oregon, and surrounding areas. The therapist will manage a caseload of patients, providing treatments and interventions to improve their ability to perform daily living activities in their home environment. The role requires independent transportation and the ability to manage a daily schedule with travel.

Responsibilities:
- Provide occupational therapy services to patients in their homes.
- Evaluate patients' physical and cognitive abilities to determine appropriate treatment plans.
- Implement treatment plans to improve patients' functional skills and independence.
- Educate patients and their families on therapeutic techniques and home exercises.
- Document patient progress and maintain accurate records in the electronic medical record system (EPIC Remote Client for Home Health and Rover).
- Collaborate with other healthcare professionals to coordinate patient care.
- Manage a daily schedule that may include travel within a 60-mile radius.
Required Skills & Certifications:
- Active Occupational Therapist license.
- Basic Life Support (BCLS) certification.
- Minimum of 1 year of skilled home health experience.
- Oasis experience.
- Valid driver's license, car insurance, and reliable transportation.

Special Considerations:
- Traveler is expected to have their own transportation.
- Facility can call off UP TO one shift per pay period (every two weeks).
- The facility cannot accommodate 4x10s or 3x12s schedules.
- Mileage reimbursement is provided at the IRS standard rate.
Scheduling:
- 8-hour day shifts.
- Weekends are not required.
- Block scheduling is not accommodated.
